How to Request Your Records
- Once logged into Clearpath, select the Request Record Tab
- To request a record, select the Request Record button on the bottom right corner
- Select Record Custodian (Provider Location)
- If you do not see Atlantic Medical Imaging’s name listed, please reach out to help@myclearpath.com
- Select records to request, body part, and date range
- Agree to the terms and conditions and submit
Prior to requesting your records, please ensure a copy of your Photo ID is attached to your profile under the My Identity Tab
Once submitted, Atlantic Medical Imaging will review and process. Once your request is processed, if approved you will see your images under the My Records tab
Tip: Be sure to upload a copy of your photo ID under the My Identity tab before requesting records.
